ALL PATIENTS - Required Items for every appointment:

  • Patient forms if it has been a year since you have been seen. 
  • Driver's license or identification card.
  • Insurance card.
  • Current medication list including medication name, strength and directions for use.
  • Referral from your primary care doctor if required by your insurance company.
  • Method of payment. We accept credit cards, personal checks, and cash.

Failure to bring these items with you will delay your medical care.

NEW PATIENTS - Required items for first appointment:

  • Patient forms. These are included in the patient forms tab. 

  • Driver's license or identification card.

  • Method of payments. We accept credit cards, personal checks, and cash.

Failure to bring these items with you will delay your medical care.

Office Policies

  • Our office visits are by appointment only. Our providers do not see patients on a walk-in basis.

  • All appointments not cancelled 24 hours in advance will receive a "no show" charge of $25.00.

  • Procedures must be cancelled with our office 48 hours prior to the procedure or a $50 fee per procedure will be assessed. Cancelling on Friday for a scheduled procedure the following Monday would be considered less than 48 hours. Leaving a message of cancellation or intent to reschedule with the endoscopy center is not adequate notice. When cancelling, please ask for the person's name with whom you relayed the information.

  • Please allow up to 3 business days for refill requests to be processed.

  • All prescription renewals require a yearly follow-up visit. Please call to schedule your yearly follow-up at least 2 months prior to running out of your prescription. Our providers are often booked out 4-8 weeks depending on the time of year.

  • If you are given a new prescription at your office visit, you MUST take this prescription to your pharmacy to get it filled. Our providers will not call in your prescription if it was written during your appointment. If we get a refill request on the prescription that was handed to you, and you have not turned it in to the pharmacy, it will be denied. You must turn in your prescription.

  • All controlled substances require a new prescription. They cannot be refilled by fax. Please call the office 5 to 7 days before you run out, so that Dr. Nunez has time to review your request for a new controlled substance prescription. Please allow 3 business days for this review. 

  • All prescription (name brand) requiring a prior authorization will be assessed a $15.00 fee. You may choose to ask the doctor to change your prescription to a generic version of a medication covered by your insurance at no charge. However, we ask that you contact your pharmacy or insurance to find out what generic medication is covered by your insurance. Every insurance policy is different, and insurance companies often update their preferred drugs. 

 

Our Wait Times

We realize our wait times are longer than some primary care physicians; however, we are a specialist office. We appreciate understanding during your visit at our office. Dr. Nunez takes a lot of time with each patient, and our schedule does not always run on schedule. This is due in part because we cannot predict what happens at the endoscopy center prior to his clinic hours. There are many variables which cause that schedule to run over the allotted time. For example, the number of polyps the patients have that day, whether a patient has a questionable lesion, cancer, and many other possibilities. Rest assured, if you are that patient who requires extra time whether at our office or the endoscopy center, you will most definitely receive it. If possible, please bring something with you to help pass the time, or you may utilize our televisions. Our staff will do our best to keep you updated. Thank you for your patience.
